- National Provider Identifier (65%), state license number and code (64.5% each), and FAX number (70.5%)
- Hospital identifier, number of beds, and web address, for facility-affiliated records
- PAC ID, professional enrollment ID, graduation year, and Medicare/eRx/PQRS participation flags (31% each)
- EHR/EMR type, certification, and vendor details, plus prescribing drug and HCPCS/CPT claims data, where available on the source record (18% to 29%)
